Doctor showing female reproductive system model to explain endometriosis and fertility care at EPIA

What is the Endometrial Immunology Test?

The endometrial immunology test is a diagnostic evaluation that checks for immune system activity in the uterine lining that could interfere with embryo implantation or lead to early pregnancy loss. Even when high-quality embryos are transferred during IVF, certain immune responses—like elevated natural killer (NK) cell activity—can prevent successful implantation.

At EPIA, this test is recommended for individuals with recurrent implantation failure, repeated miscarriages, or conditions like endometriosis. It helps us identify hidden inflammatory or immunological factors and allows for more precise cycle planning and immune-based treatments.

Who is the Endometrial Immunology Test For?

  • Individuals with two or more failed IVF or IUI cycles
  • Those with repeated early pregnancy loss or unexplained miscarriages
  • Women with suspected chronic inflammation of the uterus
  • Individuals diagnosed with endometriosis or chronic endometritis
  • Anyone preparing for embryo transfer after repeated implantation failure

Transformation

What is the Endometrial Immunology Testing Process?

  • Step 1: Clinical Evaluation and Cycle Tracking - Your doctor will review your medical history, IVF attempts, and symptoms. If immune testing is advised, your cycle will be mapped for proper timing.
  • Step 2: Endometrial Biopsy - During a specific window in your cycle, a small sample of the uterine lining is collected. The procedure is brief, typically done without anaesthesia, and may cause mild cramping.
  • Step 3: Laboratory Analysis - The sample is sent to a specialised lab to check for natural killer (NK) cell concentration, cytokine profiles, and signs of immune dysregulation.
  • Step 4: Review and Immune Treatment Planning - Once results are ready, your doctor will recommend a personalised immune support plan—this may include corticosteroids, antibiotics, blood thinners, or adjusted embryo transfer timing.

FAQs

Can't find what you're looking for?

What does this test measure?

It evaluates immune activity in the uterine lining, including levels of natural killer (NK) cells, cytokines, and other markers that may interfere with embryo implantation.

Who should consider this test?

It is recommended for anyone with two or more failed IVF cycles, multiple miscarriages, chronic uterine inflammation, or a history of endometriosis.

How is the test performed?

A thin tube is used to gently collect a sample of endometrial tissue. The procedure is quick and usually done in the clinic without anaesthesia.

Is the biopsy painful?

You may feel mild cramping, similar to a PAP smear or period cramps. Discomfort is brief and well tolerated by most patients.

Can immune problems be treated?

Yes. If the test reveals immune irregularities, your doctor may recommend medications like corticosteroids, antibiotics, blood thinners, or changes to your embryo transfer timing.

Will this delay my treatment?

Not usually. The test is often done in a prep cycle or before your next embryo transfer, so it helps improve timing without significantly extending your treatment plan.
